Sounds like your problem may be stuck\sticking EGR related, or fuel - would need a vacuum guage on the line from the filter to the CP3 inj pump to verify flow to the IP - could be failing COP valve in IP, resulting in: could be low rail pressure, also RFP sensor or\and RP Regulator solenoid - could be stopped-up fuel return line - could be faulty Baro sensor, telling ECM that the KJ is on top of Pike's peak, so idle fuelrate needs be reduced - could be faulty MAF telling ECM to reduce idle fuel flowrate - could be faulty inlet fuel temp, telling ECM to reduce fuel flowrate - could be faulty ECM, reducing idle fuel flowrate - could be needs to have up close and personal relationship with dealer scantool while this is occuring, first to check for archived DTC's which cannot be cleared with a simple readertool, then to monitor engine parameters for oddities - could be stopped-up soot-trap, or pinched exhaust\tail pipe - could be etc...............
