There are good belts and cheap belts; you can buy one or the other but not both. I like Gates belts but be aware there is a cheap Gates CRD serpentine replacement belt, like $15 or so, that you definitely don't want. Just get the Premium grade version which costs like $40. By the way if your old belt is still in good shape curl it up in a 2 gallon zip lock bag to save for future emergency use.

I think you can see it is coming apart , and the key is not in the slot

mine was coming apart like that and rubbing on the back of the power steering pump pulley

You could put the tensioner in a vice and press it back together and you will see where the bearing has slid. Comparing with new you can see also. You would think that there would be a snap ring instead of just press fit on a diesel engine that put out lots of vibration. I remember when IH had so many problems with their oil senders until they mounted it to the firewall with hoses and fittings instead directly to the engine block. When the oil sender failed the automatic shut down would kick in and we would have a 12 yard dump truck stuck on the highway full of dirt. The best thing was to replace the sender on the spot rather than unload (not so easy) and tow.

Your alternator pulley will have to be replaced too, it was the casue of all the stress placed on the tensioner and its failure.

Not the alternator coupler at all. There are two bearings on the tensioner. One for the roller for the backside of the belt and the other for the spring that hold the belt tight. Well it's the bearing that rotate with the spring that vibrated forward allowing the outer half of the tensioner to scrap up against the power steering pump pulley. Because of that interference the belt can jump off because it jams too loose and too tight. I caught mine before the belt came off but belt was damaged from rubbing the tensioner arm because tensioner was out of alignment. I still have the old one and can take pictures and post if needed.

Edit: Since I replaced my tensioner, I put 1,000 miles on it and it runs smooth. If the alternator coupler was bad I would replace the alternator for the vehicle has 100,000 miles on it. Yep I replace the timing belt too at the same time.
