 Post subject: Steam Coming out from oil cap, hi water comsuption

Dear friends:
I did a search before posting this issue, if this is been covered previously my apologies for that.

I just replace the head gasket (cylinder head was taken to a machine shop, and was pressure tested), water pump, thermostat, and timing belt on my CRD. And It still consumes water. When you open the oil cap, steam can be seen coming out, and vapor can be seen on the cap it self. No water have made contact with the oil in the oil crank, since the oil doesn't show it. My questions are:

1. Could it be possible that the oil cooler went bad?
2. Could the fixing done, been executed poorly?
3. Are there any water passage that I am unaware of, and it could be causing this issue?

Please advice!
