I don't know about 'official'--never heard of such a thing--but here's a template we've used at NorCal LOST. http://lostkjnorcal.com/Forms/Forms.htm

Word template using standard Avery biz card paper. Feel free to adapt.

I dunno why that is. I just tried in both Firefox and IE with no problems or password requests.
