A lot of steady-speed, steady-rpm driving will cause soot to build up around the actuator vanes of the turbo and inhibit their movement. My only thought, short of disassembling and cleaning the turbo, is an Italian tune-up. Take that with a grain of salt, though - if you have other issues besides stuck actuator vanes, it could detonate the turbo rather than burn the soot out of it.
(Italian tune-up = some hard acceleration. I.e., with the engine warmed up to normal operating temperature, get to a wide open freeway and FLOOR IT. Also you could get on the freeway, set your cruise at about 83, then disengage the cruise, slow down to 60, then hit the "Resume" button and the trans will downshift 2 gears and go WOT back to 83. If it is successful, you should see some smoke/ haze behind you from the tailpipe.)

This is most often connected with a dirty or non-functional MAP sensor. If you haven't already, clean the MAP with chlorine-free brake parts cleaner or with diesel fuel. Other possible causes include, but are not limited to: the turbo actually not putting out enough boost pressure (connected with your other code) a clogged intercooler if it has not had the CCV disconnected or Provented, a small split in one of the CAC hoses (removal, cleaning with soap and water - or replacement - is the only way to be sure short of an obvious fail), or an EGR/ FCV failure not allowing enough boost to enter the intake.
