You can get the Hayden via Amazon for a reasonable price and free shipping. The nylon fan is a dealer item but not too pricey. The hard part is getting the OEM fan off see

Couple of other fan removal thoughts:
1. a towel or piece of cardboard between the fan and the radiator to prevent radiator damage is a good idea.
2. the 2 bolts holding the shroud in place go into 2 nuts that are loosely capture in a plactic bracket - a) zip tie around the plastic bracket to keep the nuts fully captured and b) be sure to use the same bolts on reinstall (longer bolts can puncture radiator).
3. below the 2 bolts are 2 tabs - on reinstall be sure they engage in the slots on the radiator.
4. you will need to clip the zip ties holding AC/coolant overflow hose to shroud (redo on reinstall) - once done they can be moved out of the way. The AC one toward the engine.
5. the shroud will need to be tilted right and then left to work it out past the CAC hoses. Tight fit but doable. Reinstall is the reverse.
6. a bit of antiseize on the clutch nut on reinstall makes future removal less painful.
7. no need to make much effort to tighten the fan clutch nut on reinstall. Fan tightens on its own as it spins.
