Did you check the obvious possibility of a stuck down or partially down go pedal? I ask because I've experienced a sticky pedal, cured by cleaning a some silicone spray lube.
I will have to double check that

I just had to get to work so hopped into the other car.
now that I am work the project is on pause so I didn't need to rush here... figures.
Maybe the mat is pushing down on the pedal as things were rearranged when they put everything back, somehow got shifted - didn't think about that since I was not expecting this, and was in "get to work on time" mode.

Thanks for the logical slap upside the head to check the mats or stuck pedal.
I am not sure about the engine cut-off if brake and throttle are pressed. I do not think that is true since I have manually pushed both pedals at the same time like that in the past (not full throttle though) and the engine did not cut out.
