I went to Lowes and the only thing I bought was about a 12" piece of slotted angle iron. Usually near the hardware/bolts/etc. Cut both pieces to the desired length. Then for the rear bracket I used a dremel to cut along the seam, then used a vise/pliers to bend the ends to the "square" configuration. Paint em up if you'd like, then bolt them on. Not sure on the bolt sizes, I just took some from the "stash" in the garage. Use the existing bolts to attach the brackets to the frame...you only need to buy two bolts/nuts to attach the brake lines to the bracket.
Btw, if you have the time you can do this project anytime before the lift. No harm in doing that.
